Long Term Care Journey, What to Expect
NEED ROUND THE CLOCK CARE
You or your loved one need assistance with the daily activities of life such as bathing, dressing and eating. Your medical care needs may have become more complicated and you need the availability of 24 hour clinical care. Sometimes, these needs for long term care services are temporary, but you can stay in long term care as long as you need to. Some residents do find they are able to go back to a prior level of care.
END OF LIFE or PALLIATIVE CARE
If you or your loved one has reached the end of the aging journey, we have the resources to provide palliative and hospice care. Being surrounded by familiar caregivers and not having to move can be very calming for many clients. Our aim is to create comfort and peace at the sacred time that is the end of a life.

Manage Medications
All medication needs will be expertly managed by our in-house pharmacy and clinical staff. One of our Registered Nurses will complete an assessment of all of your loved one’s current medications to make sure everything is accurate, and will continue to coordinate with your loved one’s doctors and check in with their medications on a regular basis.
INSURANCE AND COSTS
You will need to determine payor sources for care: Depending on the type of care needed, different sources of payment may be involved. A representative from the facility or service of your choice is always available to help walk you through the costs and process. Admissions
The admission process will usually originate with obtaining you or your loved one’s medical info. We will then do an in-person assessment, usually at the client’s current residence. We want to get to know them and their family. From the assessment, a Personal Care Plan will be generated and we will get formal medical orders from their doctor. Paperwork can be done at the the time of starting services, or prior to, whatever is most convenient. Our priority is to get you or your loved one settled. Wills and Estates
Estate planning is an important arrangement for all aging people to have in place to avoid family disputes, legal fees and/or the government taking possession of assets. It’s also a chance for an aging person to make their wishes for their finances and health care known to their loved ones so everyone can be on the same page.
