The Foundation of Person-Centered Care
The Institute of Medicine defines patient-centered care as: “Providing care that is respectful of and responsive to individual patient preferences, needs and values, and ensuring that patient values guide all clinical decisions.” At Marquis, we wholeheartedly embrace this paradigm shift and add our expertise and passion by creating personalized care plans customized at each level of care. We like to divide our care plans into four areas: Mental, Physical, Social, and Spiritual.

Post Hospital Care
We have created what we call our 4 Pillars of Care approach to developing Personal Care Plans for everyone we serve.
MENTAL
Creating optimism and positivity is key to connecting mind and body for a successful rehabilitation. We include patients in designing their own recovery plans, which helps them move towards optimum independence.
PHYSICAL
Our goal is get folks back to their most independent selves as soon as possible. Our Rehab Team uses cutting edge techniques that empower patients to return to living their life.
SOCIAL
We recognize the realities that may come after hospitalization, like a temporary loss of independence and missing one’s home and friends, can be hard. We have dedicated staff to keep you connected to others during and after an aging event.
SPIRITUAL
For some patients, faith and spirituality can be a tremendous source of strength and greatly help through difficult times. We support the spiritual side of each patient as they are healing and provide resources whenever possible.
Alzheimer's / Memory Care
We have created what we call our 4 Pillars of Care approach to developing Personal Care Plans for everyone we serve.
MENTAL
Residents want to contribute and connect, and our job is to meet them where they are. Attention is shifted away from the resident’s behavior and focuses instead on how the social and physical environment can act as a positive substitute for diminishing mental processes.
PHYSICAL
We take every care to create an environment that is secure, yet open and inviting. Safe, outside spaces where they can connect to the environment are very important to our residents. Tactile, memory-triggering items, music and art are integral parts of each day.
SOCIAL
Individualization is the key. We work with residents one on one to fill their days with activities that are meaningful to them. Intergenerational programming and activities with animals have shown positive results.
SPIRITUAL
We take residents to religious services in the community as well as bring services into our facilities. Music is another way our residents express their spiritual side with programs like Music and Memory. Hospice care is also available and brought to the resident if appropriate.

Along with professional care, we bring our clients peace of mind. They are consistently engaged, challenged and objectively assessed by a person with a high level of expertise who is there to help them stay safe and get well.
PHYSICAL
Staff are trained and prepared to meet clients where they are in their healing. If coming home from a hospital or post hospital stay, the rehabilitation path that was begun can seamlessly continue.
SOCIAL
The comfort and companionship created by having consistent, trustworthy caregivers and/or clinicians is so important. Caregivers can also help keep clients connected to the things they care about by taking them to their regular social functions.
SPIRITUAL
In home caregivers can take clients to religious services and accompany them at the service if requested. We can also connect clients to our Vital Volunteer Program through our Vital Life Foundation that gives many people a new sense of purpose and connection to their community.

Our residents stay sharp and engaged through robust activities and programs for all types of people and preferences. Our communities host expert speakers and cultural programs for continued learning, as well as provide game and technology rooms for a bit of brainy fun.

We have the resources you need to monitor and manage your optimal physical health. Even the most independent residents appreciate the peace of mind of having access to our Nurse Practitioners. Keep moving with things like walking groups, gardening and access to exercise facilities.
SOCIAL
Whether you’re an introvert or extrovert, or somewhere in between, you can find your fit. Forging connections with others through shared interests like music or cooking, taking group day trips or just meeting people in our community and social spaces, is made easier by design.
SPIRITUAL
We support the spiritual side of our residents by taking residents to religious services in the community as well as bringing services in. We also have offerings for those that nurture their spiritual side through things like nature and yoga.

Whether you’re an introvert or extrovert, our activity directors can help find your fit. Long term care residents often rely more heavily on caregivers, and seeing the same faces day in and day out, helps to create deep, emotional connections between residents and staff.
