This service requires a physician’s order.
PALLIATIVE CARE
If you or your loved one has reached the end of the aging journey, we have the resources to provide palliative and hospice care. Being surrounded by familiar caregivers can bring comfort and peace to your family.

With AgeRight Care at Home, managing medications is a customizable service and we support medication management however needed. This can range from a caregiver ensuring your loved one takes their medications to arranging for a self-dispensing system. For AgeRight Care at Home clients we Care can oversee medication reconciliation and hospital orders and assess medication effects and there is always a Registered Nurse on call for additional questions.
Insurance and Costs
We believe in transparent payment systems and all services and fees are clearly explained up front. There are no hidden charges and we give clear notice of any changes in level of care. An AgeRight Care at Home representative is always here to help walk you through the costs and process. STARTING CARE
To begin care, a representative will visit your home for an assessment. We want to get to know you, your preferences and your family. Our goal is to find you the very best fit in a caregiver(s). A Personal Care Plan is generated and we will get formal medical orders if needed. We will then introduce you to your caregiver(s) and ensure that their personality and training are the right fit for you. If you have questions about what type of care you need, please don’t hesitate to reach out.
WILLS AND ESTATES
This is an opportunity for an aging person to make their wishes for their finances and health care known to their loved ones so everyone can be on the same page. Estate Planning is a critical process to avoid family disputes, legal fees and/or the government taking possession of assets.
