Your doctor decides the appropriate skilled care for your condition. And with your doctor, we develop an ongoing plan of care with you in order to meet the management goals for your medical condition, and get you back on your feet.
Skilled Nursing
Registered Nurses assess your needs and provide services requiring substantial, specialized nursing. RNs initiate preventative and rehabilitative procedures and can counsel you, your family or your caregivers about appropriate care management.Physical Therapy
Physical therapists assess your strength, balance and walking ability and modify your care plan as you show improvement toward safety and independence.Occupational Therapy
Occupational therapists assess your ability to perform activities of daily living and provide recommendations and adaptations for home safety and independence.Speech Therapy
Speech therapists assess your ability to swallow and communicate, then provide training and strategies for safe and independent home living.Social Worker Visits
Medical Social Workers, experienced in the significant social and emotional factors related to health problems, provide assessment, counseling and resources specific to your needs at home.
On call 24 hours a day, 7 days a week, our team of Care Coordinators and Registered Nurses are specially trained in community based long-term care and case management supervision.
